Mental Health Clinical Support Coordinator, Head to Health Phone Service - Country SA.

The Mental Health Clinical Support Coordinator (MHCSC) will have a background Social Work, Nursing, in Occupational Therapy or Psychology.
As a MHCSC, you will be part of a multidisciplinary team who delivering high-quality mental health intake, assessment, triage, and referral services for individuals in the community who experience mental health issues and are seeking support. The priority of the HtH IAR Service is to connect people seamlessly to the most appropriate local service to meet their identified needs, consistent with the 'stepped care' approach.
MHCSC will provide an initial identification and assessment of individuals' needs, as well as assist people requiring general information. MHCSC will have responsibility for undertaking an assessment using the Commonwealth's Initial Assessment and Referral Decision Support Tool (IAR-DST) and supporting the non-clinical team to determine the level of clinical oversight, and key decisions required in matching people with the appropriate support service.
The Mental Health Clinical Coordinators will work as part of a collaborative team approach, supported by regular opportunities to reflect upon their practice and access comprehensive training and supervision
