Position purpose

The Better Futures program supports young people aged 15-21 who have been in the care of Department of Families, Fairness and Housing's care, providing support with their transition to independence.

The program enables young people to have an active voice in their transition planning and provides individualised supports both in-care and post-care across a range of life areas including housing, health and wellbeing, education, employment, and community and cultural connections.

General responsibilities of position

  • To provide support and assistance to the young person as per program funding guidelines
  • To provide practical support to the young person to encourage them to use their own strengths, and address difficulties that are impinging on their wellbeing or functioning. To link into other support services.
  • In conjunction with the Team Leader/Manager, to implement and regularly review individual case plans.
  • To participate in case review and support with the Team Leader/Manager, other NESAY workers and other agencies currently working with the young person.
