The Organisation

For over 30 years, Carers ACT has been providing emotional and practical support to unpaid carers. As the peak body for carers in the ACT, they exist to raise the voice of carers to instigate change, and work to create a community that values and respects the caring role.

Carers ACT partners with over 3,500 carers each year, connecting them to information, advice, guidance and services and empowering them to maintain their caring role and personal wellbeing.

The Role

You will develop and deliver action plans in collaboration with a range of carers providing access to information, support and referrals relevant to their caring role. With a focus on providing service excellence and empowering carer participants, your key responsibilities will include:

  • Conducting intake assessments utilising self-reflective practice methods with the carer to identify the carer’s needs and goals
  • Developing carer support and actions plans
  • Booking services and coordinating support through external agencies
  • Facilitating emergency support when required
  • Ongoing review and engagement with carers to support and encourage them to achieve goals

Skills Required

With qualifications or experience in allied health, nursing, community services, or similar, you may be a recent graduate with placement experience or a practitioner looking to move away from clinical or shift work, or you have experience from a similar role where you’ve developed skills in making care related assessments, recommendations and decisions.

Additionally you bring empathy, the ability to build rapport quickly, and a flexible approach to address the unique needs and sensitivities of participants, together with demonstrated ability to manage your time and workload and a strong commitment to teamwork and good practice.

A current Police Check, Working with Vulnerable People Check with NDIS Participants (or willingness to obtain), and Driver’s Licence is required.
