  • Starting remuneration 59K + superannuation + salary packaging

This role will play a key part in supporting the HealthPathways Melbourne team, who aims to strengthen primary care to deliver integrated, person-centred care that is comprehensive, accessible, safe and coordinated. The focus of this team is in supporting general practice through the Quality Improvement program.

We are North Western Melbourne Primary Health Network (NWMPHN), Victoria's largest independent, locally governed and run, not-for-profit PHN, dedicated to improving primary health care in our community.

What are PHNs?

PHNs have been established with two key objectives:

  • increasing the efficiency and effectiveness of services for community members, particularly those at risk of poor health outcomes
  • improving coordination of care to ensure people receive the right care in the right place, at the right time

A typical week may include:

  • Supporting the HealthPathways Melbourne team to deliver on operational and strategic activities in line with their annual workplan including pathway development.
  • Supporting with monitoring, evaluation and reporting activities and contribute to continuous quality improvement through the sharing of ideas informed by reflective practice.
  • Promptly responding to all HealthPathways Melbourne queries including triage of the central HealthPathways Melbourne email inbox and DOT feedback portal.
  • Collaborating with the PHN workforce development team to coordinate inclusion of HealthPathways in workforce development activities.
  • Contribute to the preparation of high-quality written reports and presentations to meet program and organisational reporting requirements.

We are looking for someone who has:

  • Relevant tertiary qualifications in communications, business administration, public health, health promotion, project management, social science or health related studies.
  • Establishing and maintaining effective relationships and work collaboratively with internal and external colleagues to deliver shared outcomes.
  • Working autonomously within a supportive team environment on a range of diverse issues to meet project requirements.
  • Demonstrated skills in an administration environment, including effective written and verbal skills along with familiarity with Microsoft Office.
