About the Role

The primary role of the Case Manager, Social Health includes and is not limited to:

  • Provide culturally safe casework services to clients using appropriate and evidence based social and emotional wellbeing assessments and frameworks through all stages of service delivery,
  • Ensure a comprehensive, individually tailored and culturally appropriate case management plan is in place, inclusive of both social and physical health needs for each client allocated for support.
  • Work in partnership with other Nunkuwarrin Yunti social and emotional wellbeing and primary health care programs to ensure wrap around supports are provided to each client allocated for support,
  • Referral, advocacy and liaison activities are undertaken with external government and non-government agencies to ensure clients are provided with the best possible range of services to meet their needs.
  • Provide education/awareness presentations to the community to promote positive wellness outcomes for Aboriginal and Torres Strait Islander people as required.
  • Promote the range of Nunkuwarrin Yunti services through networking, education and presentations to ensure the continued visibility of the service and ongoing referrals from other agencies.
  • Participate in the delivery of Nunkuwarrin Yunti’s emergency relief program with other members of the TP team, as coordinated by the Senior Case Worker and/or Team Manager
  • Additional periodic or ongoing activities that may arise during the tenure of the role to meet client needs or otherwise as requested of the Team Manager.

Selection Criteria

Essential: 

  • Minimum Certificate 4 level qualification or a minimum of two years of demonstrated vocational experience in a social health care setting
  • Demonstrated understanding of the contemporary issues facing Aboriginal and Torres Strait Islander people and the impacts these issues have on Indigenous Australian societies and cultures and health
  • Ability to work within relevant legislative frameworks
  • Experience in using computer software
  • Organisational skills, self-confidence and the ability to work with general supervision to assess priorities, organise workloads and meet deadlines
  • Ability to revolve conflict or solve problems in discussion with clinical supervisor as required
  • Demonstrated ability to communicate sensitively and effectively with Aboriginal and Torres Strait Islander people, organisational staff and other providers
  • Ability to communicate effectively both written and verbally with a wide range of people from Aboriginal and Torres Strait Islander communities, government and other agencies, committees and other bodies
  • Knowledge of and demonstrated commitment of workplace diversity, workplace participation, work health and safety.

Desirable:

  • Experience working within an Aboriginal Community Controlled Health Service or Primary Health Care setting
  • Experience in undertaking social health assessments
  • Knowledge of existing social / health issues within the Aboriginal and Torres Strait Islander communities 
  • Knowledge of Aboriginal culture, social behaviour and aspirations of Aboriginal people
