Open Minds is seeking a dynamic team of Clinical Care Coordinators to join our team! Funded by the Country to Coast Primary Health Network (PHN), our Mental Health, Alcohol and Other Drugs, and Suicide Prevention (MHAODSP) stepped care services provide comprehensive, integrated, and person-centred care across Central Queensland, Wide Bay, and the Sunshine Coast.

As a Clinical Care Coordinator, you'll collaborate with the Clinical Lead and Operations Manager to provide essential services across the Country to Coast Queensland (CCQ) region. Your role will involve conducting clinical assessments, developing treatment plans, delivering psychological interventions, and managing a clinical caseload. You'll also perform risk assessments, support clients, and work within multidisciplinary teams to ensure comprehensive, client-centred care. Additionally, you'll integrate family support, coordinate care plans, and facilitate transitions to tertiary care. Upholding confidentiality and ethical standards, you'll contribute to quality improvement activities and positive client outcomes.

About you:

You are a dedicated and compassionate clinician who excels in a multi-disciplinary team environment. You adapt easily to changing circumstances and are committed to delivering high-quality mental health services.

We’re also looking for:

  • Formal tertiary qualification in psychology, social work, mental health nursing, or a relevant discipline.
  • Current annual practicing certificate from AHPRA or eligibility to register with the Australian Association of Social Workers.
  • Demonstrated excellence in mental health service delivery, with significant experience working with complex mental health presentations.
  • Experience in cross-cultural settings, especially with First Nations People, and in trauma-informed, recovery-oriented services.
  • Strong stakeholder engagement, organisational, communication, problem-solving, and analytical skills.

  • Current open Driver Licence.
  • Current Working with Children Check (Blue Card) or ability to hold one.
  • Undergo a current and satisfactory Police Check.
