About the role

Palmerston aims to improve the health and wellbeing outcomes for people experiencing AOD and/or mental health issues in the Waroona, Pinjarra and Mandurah areas through facilitating;

  • access to treatment and supports that recognise the health of the whole person 
  • better relationships between service providers to reduce system fragmentation, increase collaboration and improve the patient journey and
  • the provision of timely, holistic, person centred services that meet the unique needs of vulnerable and disadvantaged people.

As an assertive outreach Counsellor/Educator you provide individual clinical assessment, counselling and referral services to consumers as well as be involved in shared and collaborative case management and with other service providers. You will form part of the Mandurah branch team though a large portion of your time and work is carried out independently in the relevant location. Here you will develop networks, participate in community projects and provide support to the community. 

Pay Level is equivalent to Level 4 – Social, Community, Home Care and Disability Services Industry Award 2010.
