We are searching for an enthusiastic, highly motivated Primary Health Practice Improvement Partner to join our team. This hands-on role will actively engage and partner with primary care providers, such as general practice, Aboriginal health organisations and allied health at an operational level to understand health priorities and local provider needs and assist with targeted support and capability building activities.
