ADA Australia is a statewide, not-for-profit, independent, community-based advocacy and education service, supporting and improving the wellbeing of older people and people with disability. ADA Australia has nine offices throughout Queensland, with its head office located in Geebung.
ADA Australia is seeking applications for a full-time Community Connector in Toowoomba or Dalby. The role is a SACS level 5.1 position under the Social, Community, Home Care and Disability Services Industry Award 2010. The role will commence as soon as possible.
The care finder program was announced in the 2021-22 Commonwealth Budget as part of the Connecting Senior Australians to Aged Care Services measure. Care finder services will:
The Community Connector will provide specialist and intensive assistance to help people within the care finder target population to understand and access aged care and connect with other relevant supports in the community. This will involve utilising an assertive outreach approach to engage and build rapport with potential clients as well as local intermediaries, stakeholders and networks.
