UPA Home Care supports older people to live independently in their own homes by providing in-home services that effectively meet their needs, while promoting independence, wellness, choice and quality of life.
We are offering an exciting opportunity for a Home Care Coordinator to join our team.
The Home Care Coordinator works in partnership with clients receiving a home care package; to co-design a plan of care, based on the client's goals, preferences and assessed needs within an individualised budget.
The Home Care Coordinator is responsible for superior customer service and the delivery of client focused care and services that effectively meets their individual needs, while promoting functional independence, wellness and lifestyles choices within a Consumer Directed Care (CDC) environment.
The Home Care Coordinator will liaise with internal and external stakeholders to ensure positive client outcomes within resource/budget allocations.
This role is based at our Lavington office.
