Position Summary

The MACNI / Complex Needs Care Plan Coordinator will provide care plan coordination services for people receiving services through the Department of Families, Fairness and Housing, Multiple and Complex Needs Initiative (MACNI). This will include supporting increased service access for clients with complex needs across multiple service domains, such as mental health, disability, drug and alcohol and clinical services. A key component of the role includes a strong capacity to develop and maintain optimal professional relationships with the local service system, to ensure a collaborative response to the client’s needs.

Whilst the substantive responsibilities of this position are to provide MACNI services, the successful applicant may be required to undertake any NDIS services as directed.

Key Responsibilities / Deliverables

MACNI Participant and Service System Engagement

  • Provide Care Plan Coordination across the client’s care team to facilitate improved service delivery across agencies.
  • Provide leadership of the care team, be a primary contact for all care team members, disseminate relevant information and facilitate and promote communication sharing across all services.
  • Facilitate the clients improved access to services by working with key stakeholders to negotiate system barriers and work towards improved relationships with relevant services, for the purpose of achieving improved outcomes.
  • Assertively follow up care team members who have responsibility for actions as agreed during the care team meeting.
  • Undertake relevant administrative tasks required for the effective implementation of the care plan.
  • Assess care team functioning and explore possible areas for review with the care team.
  • Support the resolution of conflicts or challenges within the care team.
  • Regularly liaise with the DFFH Complex Clients Coordinator or Senior Advisor throughout the life of the care plan
  • Respond to critical incidents and lead the care team in the negotiation of risk issues, including implementing risk management plans.
  • Prepare detailed and accurate monthly, quarterly and 6 monthly reports for the DHHS Complex Needs Panel regarding the progress of MACNI care plan implementation
  • Submit reports for internal review and editing in the time frame requested
  • Present client reviews to divisional Complex Client Panels (may be required to attend in person or present on Microsoft teams)
  • Implement recommendations from the Complex Needs Panel as identified through the course of MACNI care plan implementation
  • Maintain ethical practice, ensuring integrity, credibility and respectful interactions

NDIS Participant and Service System Engagement

  • Provide high quality, professional engagement with NDIS participant’s and their families.
  • Maintain optimal knowledge of the local service system through the development of positive professional relationships, in order to provide referral and linkage to NDIS and mainstream services.
  • Support the NDIS participant (and their families where relevant) to understand their plan and ensure supports are implemented in line with the participants goals.
  • Maintain a strengths based approach to work with participants at all times.
  • Undertake the Specialist Support Coordinator role with a high degree of professionalism, ensuring Red Maple Consulting is a provider of choice for NDIS participants with complex needs.

MACNI Administrative and Direct Service Responsibilities

  • Undertake projects, assessments, training, meetings or further tasks as directed by the Director
  • Maintain all information system and privacy requirements, in the time frames requested
  • Maintain highly professional standards at all times

NDIS Administrative and Direct Service Responsibilities

  • Undertake all administrative tasks required for the effective implementation of the participants NDIS plan (including consultation with providers, emails, referrals and distributing reports).
  • Ensure timely responses to all participant and service system enquiries.
  • Maintain ethical practice, ensuring integrity, credibility and respectful interactions with participants and service providers.

Organisational Responsibilities

  • Record all support coordination hours to be claimed against the NDIS plan (ensuring 80% of hours comprises billable support coordination hours).
  • Be able to account for non-billable hours through allocation to training, supervision, relationship building, pre-planning, down time etc.
  • Undertake all NDIS reporting as detailed within the NDIS plan and as requested by the NDIS planner.
  • Ensure Red Maple Consulting policies and procedures are followed at all times, and maintain adherence to Red Maple Consulting’s Human Rights Charter, Disability Charter and Child Safe Policy.
  • Adhere to all workplace health and safety standards and follow advice regarding this provided by the Director
  • Actively participate in supervision and training as requested.
  • Record all data in information systems in a timely manner as requested.

Key Selection Criteria

Essential requirements

  1. Tertiary qualifications in Social Work, Psychology, Nursing, Occupational Therapy or a relevant Allied Health discipline, and current registration with the appropriate professional body.
  2. A current police check, working with children check and NDIS worker check (or preparedness to do so prior to commencement).
  3. Ability to provide evidence of full Covid -19 vaccination status.
  4. A current driver's licence and comprehensive car insurance.

Experience

  1. Extensive, high level experience in mental health, drug and alcohol, disability, justice and community and welfare sectors.
  2. Experience in care planning and care plan implementation for people with highly complex needs.
  3. A strong understanding of the service system and the barriers that impact the complex client group.
  4. Proven experience forming positive professional relationships with local agencies, including mental health services, homeless support agencies, drug and alcohol services, schools, Child Protection, emergency services and the forensic system – and working collaboratively to maintain sustainable service pathways.
  5. Experience managing high risk situations and proven capacity to follow organisational procedures.
  6. Demonstrated ability to manage competing demands and prioritise workload commitments in order to complete tasks within set timelines.
  7. Exceptionally strong literacy and report writing skills.
