As Rise’s Case Worker – Youth at Risk, you will operate in both outreach and centre-based settings, ensuring the successful delivery of contracts from the Department of Communities and the City of Swan.
In this role, you will provide case management support to young people, addressing issues that adversely affect their social, psychological, and physical well-being. This includes, but is not limited to, addressing the impact of family and social dynamics on the young person. Additionally, you will oversee the operations of the Stratton Youth Centre and provide support to on-site youth workers and volunteers.

This is a full-time position, working 76 hours per fortnight, Monday to Friday. You will be required to work throughout the East Metro area, including at our head office in Middle Swan, Stratton Youth Centre, as well as around the City of Swan. Shift patterns will include...

Tuesday 10:30am - 06:30pm (Stratton Youth Centre 3.00pm – 6.30pm)
Wednesday 08:30am - 04:30pm
Thursday 10:30am - 06:30pm (Stratton Youth Centre 3.00pm – 6.30pm)
Friday 08:30am - 04:30pm
