The need-to-know…

As Rise’s Case Worker – Youth at Risk, you will operate in both outreach and centre-based settings, ensuring the successful delivery of contracts from the Department of Communities and the City of Swan.

In this role, you will provide case management support to young people, addressing issues that adversely affect their social, psychological, and physical well-being. This includes, but is not limited to, addressing the impact of family and social dynamics on the young person. Additionally, you will oversee the operations of the Stratton Youth Centre and provide support to on-site youth workers and volunteers.

Typical daily tasks could include…

  • Coordinate and lead ongoing early intervention and recreational activities for young people at the Stratton Edge Youth Centre.
  • Develop and implement support and action plans for youth receiving case management services.
  • Build and sustain partnerships with relevant community organisations.
  • Provide case management support to help young people achieve their goals and desired outcomes.
  • Monitor and evaluate support plans for young people to ensure effectiveness.
  • Maintain accurate records and documentation, including detailed client case notes.
  • Address complaints and handle compliments promptly and in accordance with organisational policies.
  • Assist in the recruitment, selection, and development of staff members.

This is a full-time position, working 76 hours per fortnight, Monday to Friday. You will be required to work throughout the East Metro area, including at our head office in Middle Swan, Stratton Youth Centre, as well as around the City of Swan. Shift patterns will include...

Tuesday 10:30am - 06:30pm (Stratton Youth Centre 3.00pm – 6.30pm)

Wednesday 08:30am - 04:30pm

Thursday 10:30am - 06:30pm (Stratton Youth Centre 3.00pm – 6.30pm)

Friday 08:30am - 04:30pm

The need-to-have…

  • Relevant qualifications in Community Services and/or working towards the qualification and/or relevant experience in a related role.
  • Strong communication & interpersonal skills demonstrated through experience with young people facing complex issues and their families, friends, and advocates.
  • In-depth knowledge of the challenges and issues affecting at-risk youth.
  • Ability to organise youth events and group activities.
  • Ability to work with minimal supervision in a community setting.
  • The ability to manage and prioritise your own workload.
  • Sound computer literacy.

Appointments to this position will be subject to a NDIS Worker Screening Check / NDIS Worker Orientation Module / Working with Children Check.
