The need-to-know…

As one of Rise’s Mental Health Support Workers, you will support people with psychosocial disability, their families, and carers to develop skills and capacity to promote independence in their home and community.

Through a person-centred recovery module approach, your support will enable clients to identify and develop their goals and achieve their unique potential. Whilst maintaining clear and professional boundaries, your mentoring and role modelling will assist individuals to overcome their challenges through their journey of recovery, develop valued roles in their life, build positive relationships and achieve their goals.

A typical day might look like…

  • Travel to visit multiple different clients & provide support.
  • Work alongside people you support to achieve goals and aspirations.
  • Support people using a combination of person centred, strengths based and recovery orientated approaches.
  • Increase client’s independence by supporting their decision making and problem solving.
  • Support the person to explore, engage and build upon existing interests to enhance their social and economic participation.
  • Provide regular reports on client progress, achievements, and concerns.
  • Use client management systems to accurately record incidents, support provision and to document client notes.

Rise currently have multiple part-time positions available, with various shifts across Monday to Friday, working approximately 30 hours a fortnight with the potential of more hours. Working hours are generally between 9.00am and 5.30pm, depending on availability. You will be required to use your own vehicle to travel to clients & will be reimbursed for travel time & kilometres between clients and during a service. Travel between clients will usually be up to 30 minutes, with occasional exceptions.

The need-to-have…

Here at Rise, we nurture potential, but we do need you to have a few tricks up your sleeve including the following:

  • Qualification in Human Services (e.g., Disability, Aged Care, Allied Health, Community Services) and/or relevant experience.
  • Willing to travel around Perth metro areas to multiple clients per shift.
  • Experience working in a Mental Health Recovery model.
  • Demonstrated skills to support people to live a good life.
  • Demonstrated experience in the development, delivery and assessment of plans that support a person to achieve their goals.
  • Demonstrated ability to use initiative, discernment and judgement while maintaining professional boundaries.

Appointments to this position will be subject to a NDIS Worker Screening Check / NDIS Worker orientation Module/ Working with Children Check / requires a driving licence and reliable vehicle to use for work purposes.

Costs for new checks will be covered and/or reimbursed by Rise.
