Your new role

Sacred Heart Community is seeking a highly skilled, aged care clinician the opportunity to lead clinical teams in our residential service in St Kilda. The "My Community My Way" model is unique and provides a holistic model of care to people with lived experiences of homelessness, trauma and mental illness.

Working on a maximum term part-time basis, you will be responsible for providing clinical leadership at the facility with a key focus on resident care planning, AN-ACC, quality assurance and continuous improvement, maintaining accreditation standards and overseeing the provision of specialist nursing care to residents.

Your day-to-day

  • Undertake assessment and documentation to support AN-ACC funding
  • Supervise RNs, ENs and Care Staff and liaise with volunteers in line with the values of SHM
  • Assure all care plans are reviewed and updated according to Aged Care requirements
  • Undertake a leadership role within the the facility

We are looking for someone who

  • Can contribute to and uphold the values of the Sacred Heart Community model of care “My Community My Way”
  • Bachelor of Nursing with current APHRA registration
  • An astute clinician with experience in providing contemporary aged care for vulnerable people
  • Will develop and drive clinical portfolios
  • Will work collaboratively in a dynamic environment
  • Is cognisant of the roles of the ACQSC including SIRS and mandatory reporting.
  • Knowledge of mental health and the NDIS would be highly regarded.
  • Commitment to quality and knowledge of AN-ACC and Accreditation processes
  • Be a highly motivated leader of clinical teams including RNs, ENs and PCAs

  • Maximum term Part-Time Position, 45 hours as CCC and 15 hours as RN per fortnight. This position is expected to conclude on 6 August 2024.
