Sacred Heart Mission is a community of dedicated people working together to end homelessness and disadvantage. Our strength lies in our shared purpose and values. We welcome individuality and support each other to bring our best and whole selves to the work we do each day.
Join us in making a meaningful difference in a fun and friendly organisation with a big heart.
The GreenLight Supportive Housing Program is run in partnership with VincentCare, and Salvation Army. We support people who have experienced chronic homelessness including rough sleeping to establish a home and community connection, provide specific interventions to sustain tenancies, and will support underlying health and other issues that may impact on people staying housed.
As a Supportive Housing Case Manager, you will spend your days working with other team members including our Health and Wellbeing worker and a Peer settlement support worker to help clients to settle into housing, stay housed, and improve their quality of life. The role will see you working in the community and in people’s homes as you assist them to develop goals and navigate services that meet their needs to stay housed. You will be based in the heart of St Kilda supporting clients across the South and inner metro regions of Melbourne.
A typical day will see you working with other team members and clients, delivering case management and service coordination activities, and liaising with other agencies to support people who experience homelessness and disadvantage.
