Care Finder - Kyneton

  • Based in Kyneton

The Care Finder position will provide specialist and intensive assistance to help people to understand and access aged care services and connect with other relevant supports in the community. Key components of this role include:

  • Being active within the community, providing assertive outreach to engage with those who are not linked in with formal service systems.
  • Establishing working relationships with potential referral agents, including GP’s, aged care services, disability services, neighbourhood houses and community organisations
  • Establishing trusting relationships with older people requiring assistance to identify and access service and supports
  • Assisting people to navigate My Aged Care and the aged care service system
