Spinal Cord Injuries Australia (SCIA) is a member based national For Purpose organisation promoting independence for people with a spinal cord injury and other physical disabilities. We provide NDIS and other services to support and assist our clients to get 'back on track' and achieve their goals. Working together with our customers, our aim is to eliminate the barriers that restrict social inclusion for people with a spinal cord injury and other physical disability, developing a society which is accessible in every way. SCIA Head Office is based in Sydney with offices in regional NSW, Melbourne, Canberra, Adelaide and Perth.
We are looking for a passionate individual to join our team in Melbourne as a Specialist Support Coordinator/ Social Worker. The Community Services team currently work in New South Wales and Melbourne and assist clients access NDIS funded services under Support Coordination.
The Specialist Support Coordinator/ Social Worker will provide direct support coordination services for our NDIS customers who have complex needs and require the support of a Social Worker to access the services they require
Attend NDIS planning meetings and liaise with NDIS planners as required
Create and complete goal setting and outcome-based plans for customers
Collaborate with customers, families, providers, medical practitioners, and other stakeholders.
The Specialist Support Coordinator/ Social Worker will support SCIA’s clients with complex needs to establish positive relationships with external organisations, allied health workers and the NDIA (National Disability Insurance Agency) and promote issues that affect people with spinal cord injuries and other disabilities.
The role primarily requires positive engagement with our customers to assist them to actively identify, plan and access various supports and services available to them. Communication with clients is mostly via the telephone, travelling to client sites on occasion when essential. You may choose to blend your work location between home and the office.
This role is classified at SCHADS Award Level 4 and offers an exciting opportunity for an experienced Support Coordinator with a desire to use their Social Worker qualifications and experience to provide excellent support to our clients. As the sole Specialist Support Coordinator/ Social Workerin our Victoria team, this role is suited to someone who thrives working independently, coming together with the team member at various points for meetings and collaboration.

Develop a network of organisations and providers with whom the SCIA VIC service will work
Build a customer base to support SCIA's new Support Coordination service in Victoria
You will champion the needs for people living with disability by possessing the following experience and capabilities:
Understanding of the NDIS and NDIS participant plans
Strong analytical, problem solving and conceptual skills
Proven ability to work positively with people to reach their individual goals
Excellent documentation skills in particular case records and incident planning and reporting
Have a person centred and outcome focused approach
Be a good self-manager and thrive working autonomously while still being an active part of a team
The ability to meet KPI's in relation to billable hours
Industry relevant formal qualifications or training
Case management / support coordination experience
Unrestricted Australian driver's licence.
A Bachelor Degree or equivalent in Social Work
Certificate III or IV in Disability/Community services or relevant Bachelor Degree.
Will be highly regarded.

The position is to be worked Full-Time working 37.5 hours per week.
