South Eastern Melbourne Primary Health Network (SEMPHN) has the key objectives of:
We achieve these objectives by working directly with general practitioners, other primary health care providers, secondary care providers and hospitals to ensure improved outcomes for patients.
The seven key priorities of SEMPHN are Mental Health, Aboriginal and Torres Strait Islander health, Alcohol and Other Drugs (AOD), Aged Care, Population Health, Workforce Health Development, and Digital Health.
SEMPHN stretches from St Kilda to Sorrento to Bunyip, including the major population hubs of Clayton, Dandenong, Moorabbin, Caulfield, Cranbourne, Frankston and Pakenham.
The Access and Referral team is the point of entry into the SEMPHN funded Mental Health and Alcohol and Other Drugs Services. The team receives referrals, screens for eligibility, allocates to relevant providers or assists with alternative options. A core function of the team is to facilitate new referrals as well as referrals for existing consumers that need to step up or down into different parts of the mental health stepped care framework, ensuring that consumer needs are met as they change. These positions provide clinical oversight and utilises clinical expertise to ensure appropriate screening and monitoring of referrals.
The successful candidate(s) will be flexible and a confident self-starter, with understanding of the Stepped Care Model, Mental Health and psychosocial models of health, an ability to engage with and build strong relationships with diverse stakeholders.
The successful incumbent(s) is required to have tertiary qualifications and current registration with a relevant professional body, and relevant clinical experience, knowledge of referral pathways across the Mental Health Stepped Care Model system, as well as within the service system more broadly.
