The Position

Owned by St John of God Health Care, St John of God Social Outreach provides significant care and relief for people experiencing disadvantage throughout Australia, New Zealand and the Asia Pacific region, particularly in areas where gaps in health and wellbeing services have been identified. Social Outreach services are available to all members of the community and can be accessed free of charge or at minimal cost.

Horizon House Program provides support for young people aged 16-22 years who are in need of long-term accommodation and support to enable them to access education, training and employment opportunities. Our focus is on giving them the stability, life-skills and support they need to achieve their individual goals and ultimately, make the transition to independent living.

We are currently seeking a suitably experienced community services professional to join our Ballarat team as a Support Coordinator working at either of our Ballarat Horizon House location.

The Support Coordinator is a vital to ensuring safe delivery of care to clients and in this role, you will oversee and coordinate the operational running and program delivery at Horizon House accommodation locations, including associated transition placements.

This is a permanent full time position of 76 hours per fortnight and is a great opportunity to work alongside a professional and supportive team who are dedicated to implementing change and making a positive difference to the lives of those in need.

The successful candidate will have:

  • A willingness and capacity to embrace the Mission and Values of St John of God Health Care.
  • Tertiary qualification from a recognised University and/or TAFE College in Youth Work, Social Work or Psychology and a minimum of 5 years’ experience working with young people in a residential care setting.
  • Well-developed understanding of trauma informed care including theoretical perspectives such as loss and grief, attachment, resilience and connectedness, child and adolescent development and the capacity to apply a range of trauma informed practise responses based on these theories.
  • Sound understanding of the particular challenges involved in service delivery to young people in the homelessness sector.
  • Demonstrated ability to act with a high level of professional integrity and emotional resilience whilst working with and supporting a team to meet the needs of vulnerable young people who have been exposed to trauma.
  • Experience in program coordination and leadership of a team to successfully support program delivery.
  • High level of verbal and written communication skills and computer literacy.
  • Demonstrated ability to take initiative, work independently, and as a team
  • Strong risk management skills with effective, flexible and sound problem solving skills.
  • Participate in the out of hour’s roster as On Call Coordinator.
  • Hold a full (unrestricted) State Drivers Licence.
  • Be responsible for maintaining current First Aid and CPR Certification.
