ABOUT THE OPPORTUNITY

Exciting opportunity for a motivated Case Manager to join our Youth Homelessness Support Program.

The role will:

  • Provide regular case-management support for a caseload of clients; developing case plans that are documented in accordance with organisational policies and relevant legislation.
  • Assist clients to identify their own goals and work with them within the context and boundaries of case management, with the aim of achieving independent living and long term housing
  • Advocate on behalf of clients when necessary on matters such as income security, employment, education, health, and housing
  • Promote and facilitate where possible maintenance of links with family members and other support networks
  • Assist clients with developing links with local community services
  • Assist clients in the transition to independent housing
  • Maintain a fair and effective waiting/allocation system for vacancies within transitional housing stock.
  • Protect confidentiality, integrity, user rights and access to information for all clients in line with organisational policies and relevant legislation.

ABOUT YOU

To be successful in your application you will meet the key selection criteria, including having qualifications in social work, youth work, community development or a related discipline.

You will also have:

  • Commitment to social justice
  • Demonstrated case management experience within the homelessness or community services sectors
  • A client focused approach, with the ability to prioritise the individual needs of each client
  • A genuine interest in and commitment to caring for young people in need
  • Solution and outcome focused
