Local Area Coordination Planners (LACs) work with people with a disability and their families to build capacity and to support them to achieve their goals through local networks and accessing supports and services in their community.
We are looking for two Aboriginal LAC Planners to join our team in either the Campbeltown or Bankstown office. This is your opportunity to contribute to the health and wellbeing of the Aboriginal community in the disability sector.
Reporting to the Team Leader, this role will see you working as part of a multidisciplinary team to design and deliver plans to support and strengthen NDIS and disability services, whilst promoting and protecting the health and wellbeing of Aboriginal people in the Sydney and South West Sydney districts.
This role is ideal for a self-starter who can work autonomously and efficiently manage their time, negotiate competing proprieties and achieve weekly goals. The LAC Planner will build relationships with Participants (and where appropriate their family and kinship circle and other informal supports) to deliver an NDIS approved plan that identifies the necessary funded supports and services they need to achieve their goals. They will gather the relevant information, build and develop Participant plans for NDIS approval.
