PURPOSE AND VISION

Streetside Medics is a not-for-profit, mobile medical service led by GPs, dedicated to improving healthcare access for vulnerable populations and people experiencing homelessness.

In partnership with established homelessness charities, shelters, and support services, Streetside Medics delivers primary healthcare tailored to the unique needs of these communities.

Our vision is to achieve better health and wellbeing outcomes for those who are vulnerable or without stable housing.

POSITION SUMMARY

The Clinical Nurse Lead will oversee the implementation and coordination of the Holistic Outreach and Education Initiative for Vulnerable Communities in the Illawarra/Shoalhaven region.

This role is essential in improving health outcomes for underserved populations by addressing barriers to healthcare access, increasing health education, and fostering trust within communities.

Working with culturally and linguistically diverse (CALD) groups, Aboriginal and Torres Strait Islander Peoples, and people experiencing homelessness, the Clinical Nurse Lead will engage directly with community members and lead clinical outreach initiatives in partnership with local organisations and health services.

KEY RESPONSIBILITIES

  • Deliver culturally sensitive health education and support to address preventive care and chronic disease management, ensuring patients receive tailored follow-ups and access to clinical services such as COVID-19 vaccinations.
  • Lead mobile health clinic operations, partnering with local organisations to deliver accessible care and targeted outreach campaigns within high-need areas, fostering trust and long-term engagement with diverse communities.
  • Support the Clinical Manager on key clinical policy development and quality improvement projects including the design and delivery of relevant staff training and improvement of clinic delivery processes.
  • Build and sustain relationships with community organisations, shelters, and cultural advisors to enhance outreach efforts and ensure health information and services are accessible and culturally appropriate.
  • Collect and analyse program data to evaluate service delivery, health education impact, and vaccine uptake, using insights to refine strategies and improve health outcomes whilst monitoring and reporting on program performance.

QUALIFICATIONS

  • AHPRA Registered Nurse with current NSW driver’s license.
  • Experience in community health or outreach nursing.
  • Strong background in culturally sensitive healthcare delivery, ideally with experience working with CALD, Aboriginal and Torres Strait Islander, and underserved communities.
  • Demonstrated ability in project coordination/management or leadership roles within clinical or community settings.
  • Experience in health promotion, health education, preventive care, and patient follow-up, particularly for chronic health conditions.
  • Excellent communication and interpersonal skills, with a proven track record of building trust within diverse communities.
  • Ability to work independently and with initiative.
  • Strong organisational and time management abilities.
  • Experience with cold chain management and vaccination services desirable, but not essential.
