  • Compensation: SCHCADS Award Level 5: $47.50 - $49.65 per hour

Your Role

Make a real impact and grow your career in a supportive, collaborative environment. As a Care Coordinator, you will be at the forefront of empowering individuals with complex needs to transform their lives. In the AOD Continuing and Coordinated Care Program (CCCP), you’ll deliver high-quality, person-centered care, fostering positive change for clients impacted by substance use and other challenges.

Your Responsibility

  • Lead comprehensive assessments and develop tailored care plans with clients.
  • Collaborate with referral partners and agencies to provide holistic, wrap-around support.
  • Build referral pathways to connect clients with essential services.
  • Provide culturally appropriate care for Indigenous Australians through relevant service links.
  • Maintain detailed case notes, client records, and accurate data.
  • Offer hands-on support across housing, employment, education, and life skills.

You Will Need

  •  Relevant tertiary qualifications and significant experience in AOD and mental health care coordination.
  • Proficiency in conducting assessments using standardised tools.
  • Experience providing therapeutic interventions with empathy and professionalism.
  • Strong knowledge of community resources and the ability to work independently and in a team.
  • Commitment to client-centered care aligned with the CCCP and the Society’s AOD Model of Care.
  • Strong computer literacy, especially in data collection and record-keeping.
  • A valid NSW driver’s license.
