As a Care Coordinator, you will provide high-level, person-centered care coordination and support to clients with complex needs, including substance use issues. Your role will be pivotal in delivering the AOD Continuing and Coordinated Care Program (CCCP), offering specialist support and ensuring clients receive the best possible care.
