Your Role 

As a Care Coordinator, you will provide high-level, person-centered care coordination and support to clients with complex needs, including substance use issues. Your role will be pivotal in delivering the AOD Continuing and Coordinated Care Program (CCCP), offering specialist support and ensuring clients receive the best possible care.

Your Responsibility

  • Conduct comprehensive assessments and develop tailored care plans in collaboration with clients.
  • Partner with referring staff and agencies to provide holistic support.
  • Establish and maintain referral pathways, facilitating access to a wide range of health and community services.
  • Ensure culturally appropriate treatments for Indigenous Australians, linking them with relevant services.
  • Maintain accurate client records, case notes, and data as required.
  • Provide direct support and referrals in areas such as employment, education, daily living skills, housing, social engagement, and family/community reconnection

You Will Need 

  • Relevant tertiary or VET sector qualifications with significant experience.
  • Proven experience in coordinating care for clients with complex needs, particularly in AOD and mental health.
  • Ability to conduct assessments using standardised tools.
  • Experience in providing therapeutic interventions and working with clients in a non-judgmental, ethical manner.
  • Willingness to align with the CCCP, Society’s AOD Model of Care, and Client-Centered Service Delivery Model.
  • Knowledge of community resources and ability to work both independently and as part of a team.
  • Computer literacy, particularly in data collection.
  • A valid NSW driver's license.
