The position

The Community Inclusion Program (CIP) focuses on supporting residents of Social and Community Housing at Oaks Estate in the ACT, including rough sleepers. The program aims to enhance the independence, social and community participation of vulnerable individuals through case management and innovative approaches.

As a full time Case manager within the CIP team, you will be responsible for providing one- on -one case management support to our companions (clients). You will work, collaboratively with relevant housing providers, stakeholders, rough sleepers, and Social and Community Housing tenants in Oaks Estate to help them achieve their individual goals and foster beneficial community connection.

Funded by the ACT Government’s Community Directorate and Homelessness Services, the Community Inclusion Program aims to empower service users to make informed decisions, develop their skills and confidence, access mainstream services and positively impact their community. You will also support various projects across the organisation in line with the ACT Governments Service Delivery Schedule of Specialist Homelessness Services and wellbeing indicators.

Please note this is a Fixed Term Full-Time position (76 hours per fortnight) until 30 June 2026 with limited option to work from home.

About you

To be successful in this role you will see this role as more than just a job, bringing your proven ability to work across a range of programs and client types, with the demonstrated ability to prioritise a complex and varied case load whilst working with minimum supervision.

You will have well developed communications skills along with a proactive, collaborative mindset and a demonstrated resilience and capacity to adapt to change and sometimes challenging environments.

A minimum qualification in Certificate IV in Case Management/Mental Health or higher (Higher Desirable) with demonstrated relevant experience in substance and alcohol misuse related role and/or have extensive proven commensurate experience in a similar Case Manager position within a complex work environment including High Density Social and Community Housing.
