A healthy community. A healthy career.

Are you ready to be part of something bigger, something good?

At South Western Sydney Primary Health Network, our mission is simple. We’re building healthier communities.

Through connection and care, we tackle the healthcare challenges of our region by supporting primary health providers. And as a not-for-profit, we’re passionate about improving services to meet the diverse health needs of our region.

That really matters. And so do you.

As our highly valued Clinical Mental Health Intake Officer, you’ll use your clinical experience in mental health intake, triage and assessment to be involved in all aspects of SWSPHN Mental Health Intake team and new initiatives such as the expansion of the Head to Health Referral Spoke.

You’ll make an impact at scale by:

  • Providing mental health intake, assessment, triage, and referrals for SWSPHN mental health intake. A key aspect of this will include providing clinical oversight and support to the mental health intake team. Majority of this is not directly with the consumer but based on referral information.
  • Monitoring and reviewing referral processes and systems and where needed implementing opportunities for improvement.
  • Being involved in the development and implementation of the Head to Health Referral Spoke and ongoing improvement activities related to this initiative.
  • Using your clinical expertise to comply with protocols in relation to staff safety, maintaining client clinical records, reporting, and following clear governance in relation to clinical accountability.

What you’ll bring

To succeed in this role, you will have:

  • Relevant tertiary qualifications in Psychology, Social Work, Occupational Therapy or Nursing with current registration with AHPRA.
  • Minimum two-year experience in mental health intake, triage and assessment.
  • Project management skills and ability to develop and implement new initiatives and programs.
  • Knowledge and understanding of the South Western Sydney mental health and psychosocial sector, including referral pathways.
