Due to an increase in health program portfolios, South Western Sydney Primary Health Network has an exciting, new opportunity for an Integration and Priority Populations Coordinator to join an established and highly committed team, playing a key role in managing commissioned contracts, liaising with key stakeholder groups and advocating for the improved health of priority populations within South Western Sydney.
As our highly valued Integration and Priority Populations Coordinator, you’ll make an impact at scale by:
Working with commissioned service providers to deliver real health outcomes for our local communities.
Engaging with key stakeholder groups in primary care, public health and the NGO sector to support an integrated health system.
Supporting SWSPHN’s health planning via advocacy of health needs and service gaps within your priority population areas.
To succeed in this role, you will have:
Skills and experience in contract and/or project management
Good interpersonal and stakeholder management skills
The ability to manage multiple tasks simultaneously and autonomously.
Tertiary qualifications in a relevant field (eg: health, social work, project management), or equivalent work experience, or a combination of study and work experience.

Contract until 31 December 2025 with possible extension dependant on funding.
