We have a great opportunity for highly motivated; outcome focused Case Manager to join our Alternative Learning team on a full-time permanent basis, located in our Studio North (Elizabeth) site but you may be required to travel to our Studio West site (St Clair).

Reporting to the Program Manager of Learning you will provide options and supports to inspire and challenge students with identified barriers to learning and contribute to improving the achievement of students in the focus areas of living, learning and transition.

In this role, you'll provide options and supports to inspire and challenge students with identified barriers to learning. Day to day activities will include:

  • Providing client services within SYC’s Case Management model.
  • Supporting delivery of learning program working collaboratively with teachers and other Case Manager’s.
  • Working to achieve outcomes specified in funding contracts and within program guidelines.
  • Working collaboratively with colleagues and stakeholders to undertake appropriate entry screening for clients.
  • Conducting holistic assessments of young people to establish their needs, barriers and goals.
  • Developing a case management plan with young people within funding contracts / guidelines.
  • Providing direct service including therapeutic interventions to assist young people to meet their case plan.

About You

As someone who enjoys working in a highly collaborative team environment, you will be passionate about supporting young people to engage in an alternative learning space. The role is multi-faceted including maintaining and developing relationships with schools, other relevant stakeholders, and participants to promote and achieve success on their educational journey!

To be considered for this role, you will demonstrate:

  • Experience in engaging and working with young people who are disadvantaged by homelessness, an offender history, family breakdown, poverty or lack of education.
  • Previous experience in conducting client assessments, developing case plans and writing case notes and reports.
  • Strong engagement skills and the ability to interact effectively and sensitively with young people from diverse.
    backgrounds, their families, and other service providers to achieve positive outcomes with clients.
  • Demonstrated experience in delivering group programs.

For this role, you will need to:

  • Hold a minimum of one or more of the following qualifications: Certificate IV or Diploma in Youth Work or Youth Justice, or Bachelor of Social Work or Psychology, or Certificate IV in Child, Youth and Family Intervention (Child Protection).
  • Be willing to complete a Working with Children Check and National Police Check.
  • Have possession of a current valid Driver’s License, willingness to drive, and your own registered vehicle with a 4–5-star ANCAP safety rating.
  • Be eligible to work in Australia.
