About the role

As our Care Connector, you will play a vital role in providing specialised and intensive assistance to our vulnerable and isolated older people, ensuring they can access essential services and connect with vital community supports. We aim to improve integration between health, aged care, and other systems at a grassroots level, paving the way for seamless access to the resources individuals need to thrive!

Duties

  • Engage and establish rapport with potential clients and local intermediaries, guiding them through the My Aged Care screening and assessment process.
  • Assist clients in understanding and navigating the array of aged care supports and services available, including financial considerations and provider coordination.
  • Conduct periodic check-ins with clients to ensure ongoing service satisfaction and address any changing needs or lapses in service provision.
  • Collaborate with local agencies to develop robust referral pathways and to identify potential clients who are reluctant to engage with services.

Skills and Requirements

  • Possess relevant qualifications or experience in social work, human services, aged care, community services, or health.
  • Forge local community connections.
  • Demonstrate a comprehensive understanding or the ability to quickly grasp the range of aged care supports and services available.
  • Commitment to delivering person-centred care, honouring individual needs, preferences, values, and life experiences.
  • Effective communication skills to support diverse backgrounds, including Aboriginal and Torres Strait Islander, Culturally and Linguistically Diverse (CALD), and LGBTQIA+ communities.
  • A current Class C NSW driver’s license
  • Demonstrated computer skills including Microsoft Office and ability to enter information correctly into databases and/or portals.
