We are currently recruiting for Care Coordinators, to support young people with mental health issues in clinical and non-clinical services.

Excellent opportunities are available for Mental Health Care Coordinators. You will be skilled and capable in the delivery of community or clinically based mental health services. You will be an integral member of the multidisciplinary team providing both one on one and group interventions and supports to consumers and their families living with severe and persistent mental health illness. You'll also be a leader/mentor to junior team members.

This role will include:

• Coordination of support services, risk assessments, mental health examinations and family supports.
• Manage a small case load of more complex clients
• Work within a multidisciplinary team in a non-clinical and clinical service
• Encourage and support education of family members
• Work collaboratively with internal and external agencies
• Actively be involved in planning, implementation and evaluation of programs services and therapeutic activities.
• Deliver evidence-based brief interventions, including group interventions, with young people and their families/carers;

You must have:

• Bachelor of Social Work - eligibility or registration with AASW is a must
• 2 years previous experience as a Care Coordinator or another senior position
• Sound discipline knowledge gained through experience, training or education
• Understanding of the Strengths Based Approach principles
• Knowledge of the National Mental Health Standards
• Professional or specialised knowledge in Mental illness, youth, drug and alcohol and homelessness.

You will also be willing to undergo police and Working with Children Checks, as well as providing 2 recent managerial references.

We will not be considering any applications without sufficient Youth Mental Health experience.
