About the role

We are seeking a values-driven and proactive Case Manager to join our team! You will be responsible for providing immediate and ongoing support for clients experiencing homelessness, through provision of a range of intervention options through case management support.

The Open Door and Flagstaff are residential facilities that provide temporary short-term accommodation for men who are experiencing homelessness, with multiple and complex needs. The Open Door accommodates 37 single men and Flagstaff supports 64 single men.
Reporting to the Team Leader, this is a permanent full-time position, based in Flagstaff, VIC. Salary and conditions are in accordance with SCHADS Award Level 5.

You will successfully

  • Develop and review individualised case plans with clients and assist them to implement strategies to achieve their goals by building on their strengths
  • Deliver high-quality service maintaining constructive relationships with clients by working within Person-Centred, Strengths Based and Trauma-Informed models of care
  • Support clients to access long term housing by supporting them to move on from short-term/medium-term accommodation
  • Ensure client data is accurately recorded on the client database (SAMiS) to inform program planning and meet data recording requirements of the DFFH
  • Undertake identified training and professional development activities to support practice and skills development

You will have

  • Degree qualification and higher educations in Social Work, Psychology, or related field
  • Relevant experience in the social service sector including case-management
  • Able to demonstrate knowledge of homelessness including context, causes, models, and approaches relating to best practice
  • Experience working with internal and external stakeholders to both maintain and create new connections in the sector
  • Current Australian Driver's Licence is required
  • Valid Working with Children's Check
