Why this role matters

The Salvation Army Australia's Homelessness Stream provides high quality, person centred services to people at risk, and or experiencing homelessness. We support adults and families, including accompanying children with accommodation, case management, outreach support, financial assistance, connection and referral to other specialist services.

About the role

We are looking for a values driven Case Manager who will be apart of a collaborative case management and community of support team that provides support to individuals engaged with the service. The Case manager provides person centred and trauma informed supports ensuring access for clients to relevant services, advocacy in obtaining safe and secure housing, referral in relation to legal issues, income security, health issues, drug and alcohol concerns, specialised counselling and psychiatric concerns.

The Case Manager provides case management to individuals both within our residential facilities and in an outreach capacity. Case managers work collaboratively with clients and staff to ensure they are provided with appropriate support in their journey out of homelessness.

You will successfully

  • Provide case management support to clients in accordance with TSA case management models and standards of practice including the use of the case plan tool and active participation in client care team meetings.
  • Create transferable relationships; promote independence and sustainable pathways out of homelessness.
  • Incorporate assertive engagement and rapport building with clients.
  • To develop and maintain strong relationships with appropriate services to maximise client access to mainstream and specialist services.
  • Accurate, up to date client records and data is maintained for all clients in accordance with TSA standards and procedures using TSA information systems.
  • Actively contribute to building a strong culture in the team and across the service based on mutual respect and collaboration.

This is a full time, permanent position, based in West Melbourne, VIC. Salary and conditions are in accordance with the SCHADS Award 2010 Level 5.

You will have

  • Tertiary qualification in social work or a related discipline is considered desirable
  • Experience working as a Case Manager and/or within the homelessness sector
  • Current Australian Driver's License is required
  • Proof of Covid vaccination is required
  • A National Criminal History Check is required
