About the role
We are seeking a values-driven and proactive Clinical Intake Coordinator to coordinate the acceptance of referrals received for Geelong withdrawal unit facilitating the efficient entry of clients into the service.
- The Geelong Withdrawal Unit provides a residential medicated withdrawal service for those who are seeking to recover from addiction to alcohol and other drugs. Clients reside in the Unit during their episode of care and are provided with all meals and facilities
- Nursing staff are employed to cover the 24-hour period with clinical supervision provided by the Nursing Coordinator and Visiting Medical Practitioner
- The service is funded by the Victorian Department of Health and Human Services
You will successfully
- Ensure all clients are linked into appropriate services, GWU or other, in a timely manner following receipt of comprehensive assessments for all designated clients
- Provide clients with brief interventions during this process as needed
- Address all phone calls and emails related to Intake and manage Intake Waiting Lists
- Identify any risks to clients, staff and TSA and brought to the attention of the Nursing Coordinator as soon as is possible
- Undertake appropriate preventative action to address workplace hazards as per TSA procedure
- Report incidents in the required format to the Nursing Coordinator
- Identify service gaps and are made known to the Nursing Coordinator
- Participate in the on call roster
- Maintain confidentiality of all service users with appropriate consent obtained and documented
- Identifies and makes know to the AOD Manager service gaps
- Maintains the bed register for GWU
This is a permanent full-time position based in the 12-bedroom Withdrawal Unit Corio, VIC. Salary and conditions are in accordance with Nurses & Midwives (VIC Public Health Sector Agreement 2016-2020) RN Level 3 Award.
You will have
- An understanding, empathy, appreciation and the ability to promote the values and ethos of The Salvation Army
- Current unrestricted registration with Australian Health Practitioners Regulation Agency
- Demonstrated AOD experience, the ability to work as part of a team and promote education and leadership in aged care
- Understanding of and commitment to continuous quality improvement
- Demonstrated organisational and time management skills
- Demonstrated well developed computer and documentation skills
