Role Overview

The headspace Continuing Care Team (CCT) Case Manager role involves the provision of comprehensive, evidence-based case management consistent with the EPPIC (Early Psychosis Prevention & Intervention Centre) model of care.

This Case Manager aims to support the treating team to make the health system more accessible for young people registered within the service by assisting with ensuring young people are linked into all appropriate services as required. 

The role provides support by way of information, resources and pathways, assisting young people to gain access to services in line with care plans. 

The role is responsible for the intake & assessment of young people presenting to the service, determining suitability to the program and to co-ordinate care.  

Essential Criteria:

• Tertiary qualifications in Psychology, Social Work or Occupational Therapy and AHPRA membership or AASW eligibility
• One year or more experience working within clinical, health and/or welfare service and support systems.
• Ability to deliver intake, triage and biopsychosocial assessment, risk assessment, case formulation and therapeutic interventions
• Experience working with a diverse range of health professionals and young people.
• Ability to complete referral forms, needs assessments and outcome measures for young people.
• Understanding of mental health legislation.
