About the Opportunity

At Uniting, our Case Managers are called Community Support Partners. This role is responsible for the case management of Care Finder Program clients who require intensive support to access aged care services and other local community supports. The Community Support Partner will utilise specialist strategies that are person-centred, to produce outcomes that enable clients to effectively participate in decision making related to their care, access services, and to keep them engaged in their care journey. 

 Fixed term, Full Time contract until June 2025.

 About you

  • Tertiary or Diploma qualification in a relevant field, such as Aged Care, Health, Business and/or working towards or equivalent years of experience
  • Current Australian Health Professional Registration Authority (AHPRA) and/or Case Management Society of Australia and NZ certificate, in line with qualification/s
  • Experience in the Aged Care and/or the Health environment
  • Significant understanding for client needs, to convert these into positive service delivery outcomes
  • Confident in building and maintaining strong working relationships with internal and external stakeholders
  • Passion for social change and contributing to an organisation of influence for vulnerable, at risk and disadvantaged members of the community
  • Proficient in using multiple computer applications, as well as the willingness to learn new systems/programs
  • First Aid Certificate
  • Current driver's licence and access to a comprehensively insured vehicle
