What You’ll Be Doing

 In this role, you will have the opportunity to lead and deliver high-quality clinical care through a case management approach, ensuring each client receives the attention and expertise they deserve. Your day-to-day will involve:

• Coordinating and delivering comprehensive clinical care using assessment-driven strategies to enhance the well-being of our clients.
• Collaborating closely with clients, multi-disciplinary teams, and external partners to deliver integrated care solutions that meet shared goals.
• Fostering a supportive team environment where respect, integrity, and the exchange of ideas are celebrated.

Who We’re Looking For

• Previous nursing experience or a passion for entering the aged care sector.
