Home Support Assessor - Port Lincoln

The Home Support Assessor is responsible for undertaking assessments for people accessing the Commonwealth Home Support Program (CHSP) within the My Aged Care program. Role located in Port Lincoln.

  • Undertake Home Support Assessments and develop client support plans for older people, including people from CALD and Indigenous backgrounds, via the My Aged Care Portal utilising the National Screening and Assessment form (NSAF)
  • Work with clients, their families and carers to identify support options including both CHSP and non-funded service and make appropriate referrals based on client identified goals.
  • Recognise and respond to opportunities for reablement where possible 
  • Meet all assessment and support plan KPIs as specified by the Department of Health.
  • Accurate and timely recording of client data in the My Aged Care Portal and internal reporting mechanisms 

For this role there will be a requirement to have SA02 (Aged Care Assessment Training) or be willing to undergo once employed with Uniting Communities.

About You:

  • A working knowledge of the Home and Community care sector
  • Experience working with older people and ability to demonstrate an understanding of the physical and psychological aspects of aging
  • Experience using computer based portals
  • Experience working in rural and remote communities
  • Experience working with Indigenous clients in rural settings particularly advantageous 
  • Regular intrastate travel including but not limited to travel to other major regional centres, surrounding towns and localities and remote communities
