About the role 

This is an exciting opportunity to join Uniting SA’s Community Aged Care team at West Lakes as our Community Registered Nurse. You will focus on developing and implementing support plans as well as delivering services to clients transitioning from acute services through our Transitional Care Program as well as support and advocate for our Home Care clients. 

Key Responsibilities

  • Under the direction of the Clinical Program Manager, deliver quality clinical care services to consumers according to their assessed needs, goals and preferences.
  • Undertake or delegate clinical assessments for all new Transitional Care Program (TCP) admissions in a timely manner.
  • Provide clinical input/assist in the development of HCP and TCP client health and wellness assessments.
  • Use a consumer focussed approach to care and service delivery.
  • Respond to changes in a consumer’s condition in collaboration with Clinical Program Manager, multidisciplinary team and others as needed.
  • Provide clinical guidance, professional support and advice on client and care related issues, with oversight from Clinical Program Manager.
  • Ensure infection control practices are embedded within the team.
  • Ensure care documentation is timely, accurate, succinct and relevant
  • Educate consumers and their families on diseases, prevention and control measures in a way that is tailored to the communication needs and understanding of the individual/s 

Skills & Experience

  • Recent experience as a Registered Nurse (RN) in an acute or community aged care setting.
  • Able to plan and coordinate the clinical management of care under direction of a Clinical Program Manager.
  • Able to educate consumers and their family regarding clinical aspects of care.
  • Ability to communicate sensitively, openly and effectively with consumers, their families/carers, key stakeholders, and other staff.
  • Ability to apply knowledge and skills to a broad range of complex tasks.
  • Ability to follow organisational work instructions, and policies and procedures
  • Ability to build and maintain professional relationships with clients and service providers.
