About Your Next Exciting Opportunity!

Your new role will be to manage the Aged Care Service delivery program in all service areas whilst providing strong leadership to a team of Aged Care Specialists. This will include close partnership with state health in warm referral pathways and access to the My Aged Care Portal.

The Coordinator Aged Care Services is a new opportunity with Wellways and a role that exists to coordinate the development, implementation and ongoing delivery of the Aged Care Specialists that is part of the Carer Gateway Services. The new Carer Gateway Services have been created in the context of national reforms and represent an exciting new era for the delivery of innovative carer services.

The Carer Gateway Services consist of 6 unique service types:

  • Carer Support Planning: Intake needs assessment and planning
  • Carer Coaching
  • In-person peer support
  • In-person counselling
  • Carer-directed support packages
  • Emergency respite care

Your experience within the aged care sector will be an asset to Wellways and your understanding of people management will assist you greatly to advocate for and lead the implementation of the carer peer workforce, prioritising the employment of carer peers at every point.

So What Will You Need?

  • Tertiary Qualification in a relevant field of study and minimum 3 years'
  • leadership experience
  • Experience in high volume operational environments
  • Current valid Driver's License
  • High computer literacy skills
  • Satisfactory National Police Records Check
  • NSW Working With Children Check
  • NDIS Workers Screening Check
  • NDIS Orientation Module
  • Right to Work within Australia
  • Availability to participate in out of hours activities as required to undertake the requirements of the role
  • FULL COVID19 Vaccination Certificate (including booster if eligible) or valid medical exemption
