We are seeking x4 Carer Support Planners to join our supportive team at Wellways, within CarerGateway Services. These roles will be based from our Campbelltown office in NSW, and will suit individuals who have exceptional customer service and communication skills and enjoy a fast paced environment.

About Your Next Exciting Opportunity

The Carer Support Planner is a newly created role responsible for undertaking intake, assessment and planning services for Carers accessing support provided by the Carer Gateway across New South Wales. The objective of the role is to work with Carers primarily over the phone to access information, support and referrals relevant to their caring role.

The new Carer Gateway Services have been created in the context of national reforms and represent an exciting new era for the delivery of innovative Carer services. Carer Gateway Services consist of 6 unique service types:

  • Carer Support Planning : Intake needs assessment and planning
  • Carer Coaching
  • In-person peer support
  • In-person counselling
  • Carer-directed support packages
  • Emergency respite care

A typical day in this role will involve supporting carers with a high level of customer service ensuring carers are listened to with patience and non-judgement. You will provide Intake, Planning and Assessment services to Carers and work with carers to establish and implement agreed action plans.

So What Do You Need?

  • Contact Centre Experience - Highly desirable
  • Qualifications and/or experience working in community services sector - desirable
  • Appropriate IT skills
  • Right to work in Australia
  • Satisfactory Police Records Check (no older than 12 months)
  • NSW Working with Children Check prior to commencement
  • Ability to complete NDIS Orientation Module Certificate prior to commencement
  • NDIS Worker Screening Check
  • FULL COVID vaccination certificate (including booster if eligible), or a valid exemption from a registered medical practitioner
