We are seeking three Carer Support Planner's (Emergency Respite specialised) to join our supportive team within Carer Gateway Services at Wellways Australia. These roles will be based from our Campbelltown office, NSW and will suit individuals who are looking to work set shifts between 12pm - 8pm.
The Carer Support Planner (Emergency Respite) is a newly created role that exists to implement the delivery of the emergency respite program that is part of the Carer Gateway Services.
Working in the role of Carer Support Planner (Emergency Respite), you will be responsible for implementing emergency respite care and assisting carers to access the Carer Gateway Services program. Reporting to the Coordinator, you will be comfortable to work under limited direction.
A typical day in this role will involve supporting carers with a high level of customer service ensuring carers are listened to with patience and non-judgement. You will provide emergency Intake, Planning and Assessment services to Carers and work with carers to establish and implement agreed action plans. All contact with emergency providers is conducted over the phone.
