Youth Community Living Support Services (YCLSS) delivers time-limited specialist community based psychosocial support services designed for young people aged 15 to 24, with serious mental illness, who have, or are at risk of developing functional disability due to their mental health. This program works in close partnership with Local Health Districts, with a focus on coordinated and integrated care. Wellways YCLSS program offers young people flexible hours and types of support to ensure they meet current needs and goals.
At Wellways the YCLSS program services are delivered within the context of a recovery model and in a way that empowers individuals to live in the community with self-determination and independence.
The primary focus of this program aims to support the personal recovery of young people with mental illness through tailored community psychosocial support. The aim of the YCLSS program is to minimise the risk of young people developing chronic disability and requiring frequent hospital admissions, long inpatient stays and long-term care.
The Team Leader oversees the day-to-day operations of the YCLSS program. Your main focus will be on providing supervision and coaching to YLCSS staff that ensures the delivery of high quality, safe, effective and evidence-based supports under the direction of the Program Coordinator.
The Team Leader carries a small YCLSS caseload, working alongside participants, providing intensive support that help the participant transition into adulthood: social skills, decision making, healthy lifestyle, responsibility, self-identity, self-esteem, and resilience.
