About the Role

Your new role – In this role, in The Way Back Support Service, you will provide holistic non-clinical follow-up care, practical support & assertive outreach to people directly after a suicide attempt or at high risk of suicide. This is a unique role that will require you to provide practical psychosocial support to participants through assertive outreach.  Support Coordinators link clients to existing clinical and community-based services, to ensure they are safe and accessing the community-based support that is available. You will work collaboratively to:

• Improve access to high-quality aftercare to support at risk individuals to stay safe.
• Build capacity of individuals to self-manage distress and improve mental wellbeing;
• Improve links with clinical and community-based services to meet individual needs and circumstances
• Increase social connectedness and links to supportive networks (families, friends, peers and carers)

What you'll bring to the role

• Passionate about making a positive change in the lives of people managing suicidality
• Tertiary qualifications in community, social welfare, health or a related discipline
• Ability to connect with people and offer them support to identified needs
• Capacity to work independently and collaboratively within a team environment
• Current Driver's licence and willingness to drive in the course of work
