Our team centres on social justice and is passionate about ensuring that everyone with a disability is able to gain access to the appropriate resources to achieve their goals as our Support Coordination Manager you will plays a crucial role in achieving this.

About the role:

Windgap currently has a Permanent Full Time (38 hours per week) position available for a qualified Social Worker, Located in Banksmeadow. This is a SCHADS level 6 ($51.90) position. As the Support Coordination Manager, you will lead and oversee a team of Support Coordinators, who are responsible for assisting participants with Support Coordination in the National Disability Insurance Scheme (NDIS) plan to connect to and coordinate with a range of informal and mainstream supports that align with their funding and goals.

Your primary focus will be on ensuring efficient and effective support coordination services are delivered by the team to meet the needs and expectations of our participants. You will play a pivotal role in enhancing team performance and driving continuous improvement across Support Coordination operations.

As the Support Coordination Manager, you will have the opportunity to make a significant impact on the lives of our participants by ensuring outstanding service provision to your participants and participants across the team.

Your key tasks will include:

  • Team Leadership and Development – provide strong guidance and support to the team to ensure excellent service delivery to all Support Coordination participants.
  • Acting as a facilitator between clients in need and community services
  • Assessing resources for health, welfare, recreation, housing, employment and other community services
  • Conducting individual and family case interviews to identify the nature and extent of client’s problems
  • Assisting clients to understand and resolve problems by providing information, acting as a mediator and referring them to community and self-help agencies
  • Providing capacity building support in order to strengthen an individual’s ability to coordinate and implement their NDIS funding
  • Providing a safe and supportive environment that allows clients to navigate points of crises and receive appropriate support
  • Analysing, developing, promoting and implementing social policies through the use of practice experience, research, analytic frameworks, and negotiation skills to respond to social need through a fair, equitable and effective allocation of social resources
  • Monitoring the progress of clients by maintaining contact
  • Compiling case records and reports

What you need to succeed?

You’ll need to demonstrate:

  • Bachelor's degree, or higher in Social Work or a related field.
  • At least 2 years relevant experience
  • Knowledge and understanding of the National Disability Insurance Scheme (NDIS) and the disability sector
  • Previous experience supporting people with disability and knowledge of community services
  • Comprehensive report writing skills and strong attention to detail
  • Demonstrated experience applying assessment tools to identify support needs
  • Demonstrated computer skills, including MS Word, Excel and PowerPoint, Outlook with experience recording progress notes
  • Proven experience in a customer support or customer service management role.
  • A passion for delivering exceptional customer experiences
  • Current NSW driver’s licence.
