About the Program

The Wesley Care Finder Program is designed to assist older people in accessing and understanding the aged care system. We provide support to connect participants to relevant programs and services in the community, targeting individuals who face barriers such as:

  • Isolation or lack of a support person.
  • Communication challenges, including literacy barriers.
  • Difficulty processing information or making decisions.
  • Hesitancy to engage with aged care services or institutions.

This program is funded by the Primary Health Network (PHN) Hunter New England and Central Coast.

About the Role

We are seeking an experienced Intake Support Worker to join our team in Maitland. This role is vital in ensuring a professional and welcoming first point of contact for clients while supporting the team with key administrative functions.

Key Responsibilities:

  • Handle incoming inquiries and manage client intake processes.
  • Maintain accurate client records and manage data entry in CRM systems.
  • Conduct initial assessments to determine client eligibility for the Care Finder Program.
  • Provide trauma-informed and culturally appropriate intake support.
  • Support team operations with general administrative duties and referral management.

Selection Criteria:

To be successful in this position, candidates must possess the following:

Essential:

  • Experience working in community or client services.
  • Proven ability to deliver high quality client support.
  • Demonstrated experience in providing trauma informed care and culturally appropriate practice
  • Demonstrated commitment to supporting the needs and rights of people from diverse backgrounds, e.g., Aboriginal and Torres Strait Islander communities, Culturally and Linguistically Diverse communities, people from Lesbian, Gay, Bisexual, Transgender and/or Intersex communities, and those from rural/remote communities.
  • Excellent verbal and written communication skills, with the ability to engage and relate to clients, referrers, and colleagues from diverse backgrounds.
  • Strong active listening skills with a demonstrated ability to respond empathetically and professionally to client inquiries
  • Proven ability to accurately and efficiently record client information in a CRM or similar database, ensuring that all client details are kept up to date.
  • Strong attention to detail with a commitment to maintaining data integrity
  • Ability to identify client needs and recommend appropriate services or solutions.
  • Proactive in addressing challenges and offering solutions in line with the care finder program
  • Display emotional maturity and resilience, maintaining professionalism in challenging or stressful situations.
  • Ability to stay calm and manage conflicting priorities while maintaining a positive outlook
  • Ability to work independently and collaboratively
  • Proficiency in office software and data system
  • Willingness to affirm Wesley Mission’s vision, mission and values
  • Successful applicant will need a Working with Children’s Check & National Criminal Record Check.

Desirable

  • Tertiary qualification, in a related field, e.g., aged care, community care, social work, allied health, nursing and/or demonstrated professional experience working with clients to deliver a community-based service within the aged care sector.
  • Familiarity with local community resources, including aged care services and support networks.
  • Experience collecting and analysing data for program evaluation and contributing to the development and improvement of services and programs
