Your new role

As a Case Manager - Out of Home Care, you are responsible for providing support to children and young people aged 0-18 years and their families in a foster care, kinship or kith care setting.

In this role, you will provide direct case management to children and young people from a variety of out of home care programs and facilitate referrals to access appropriate support services where required.

You will also provide further resourcing to our Foster Care team through carer recruitment and retention activities and may assist in supporting accreditation and assessment of foster carers.

Your skills and experience

  • A minimum of a Diploma level qualification in Community Services or equivalent
  • A tertiary qualification in Social Work or equivalent is preferred
  • Current Australian driver’s license
  • Prior experience in a case management role
  • Demonstrated experience in engaging with families successfully and completing assertive outreach.
  • Demonstrated ability to assess and appropriately respond to risk for children aged 0-18 years
  • An understanding of the ‘Best Interests’ Case Practice Model and Children Youth and Families Act (CYFA) 2005 and 2015 amendments.
