About the opportunity

We have an opportunity for 3 Case Managers to join our Navigator Program based at Blackburn on a full-time basis. The Navigator Program is delivered in partnership between Uniting, VACCA and the Australian Childhood Foundation. The program utilises a trauma informed, culturally safe assertive case management model to respond to challenges experienced by young people aged 12–17 years with the goal of re-engaging disengaged learners in education.

The program utilises a Capability approach that understands how multiple characteristics can interact to compound educational disengagement that can result in disconnection and a lack of a sense of purpose and self-worth for young people. This amplifies service barriers and exacerbates social and economic disadvantage, and social isolation.

Working collaboratively with partner agencies and funding bodies, the Case Manager will provide direct support to a caseload of disengaged learners. The role will undertake comprehensive assessments, establish and lead a care team and work together with children and families to establish individualised education re-engagement plans.

About you

To be successful in your application, you will meet the person specification in the position description, including having a relevant tertiary qualification in Social Work, Psychology, Child Development/Community Welfare, or other equivalent:

  • Demonstrable experience working with vulnerable young people and families
  • Knowledge of family services, child protection and the department of education and training, including relevant legislation and regulations
  • Knowledge and understanding of trauma informed and culturally competent practice, including knowledge and application of therapeutic parenting practices
  • An understanding of the barriers experienced by disengaged learners
  • Proven ability to develop strong relationships with key stakeholders and develop and implement a new program initiative
