We have an exciting opportunity for a social worker to join our Keeping Connected Sibling Support and Placement Service Program. The service provides flexible support to vulnerable sibling groups in out of home care, particularly in foster and kinship care arrangements. The program aims to keep sibling groups together in a home based care setting and prevent children and young people from being placed in residential care unnecessarily.
The Social Worker is responsible for developing child focused and safe contact plans for sibling groups who reside in out of home care, but do not live together. The role will provide assessment, information and advice to care teams to achieve desired outcomes for children and young people.
